Recent data reveals a concerning trend: over 22,000 residents of Alabama have lost their health insurance coverage obtained through the federal Affordable Care Act (ACA) marketplace in the past year. This decline has sparked urgent conversations about healthcare accessibility in the state, particularly for those who can’t afford rising insurance premiums.
Debbie Smith, the campaign director for Cover Alabama, a group advocating for Medicaid expansion, expressed her concern: "We’ve heard many heartbreaking stories of Alabamians who saw their health insurance costs soar to an amount they simply can’t afford after losing their premium tax credits. This enrollment drop demonstrates that a significant number of individuals are faced with painful choices regarding their healthcare."
Fortunately, the Alabama Department of Public Health has provided a lifeline for uninsured residents. Here are several types of services available:
Federally Qualified Health Centers: Across Alabama, numerous community-based nonprofit health centers receive federal funding to deliver comprehensive medical services to everyone, regardless of their insurance status. You can find a complete directory of these centers here.
County Health Departments: The Alabama Department of Public Health offers essential services at local county health departments. These include immunizations, tuberculosis treatment, and family planning services. Notably, they also offer free breast and cervical cancer screenings for women who either lack insurance or earn an income at or below $39,125 annually for individuals. Remote Area Medical: This organization hosts pop-up clinics throughout Alabama, providing high-quality care free of charge, including dental and vision services. The recent decrease in ACA marketplace enrollment aligns with the expiration of enhanced tax credits that significantly lowered insurance costs, which ended on December 31. According to the Kaiser Family Foundation (KFF), if these credits are not reinstated, it’s estimated that around 130,000 Alabamians could lose their health coverage, as premiums in the ACA marketplace are projected to rise by an alarming average of 93% statewide.
Last year, nearly 10% of Alabama's population was enrolled in the ACA marketplace, a significant increase from 2020, when just over 160,000 residents were signed up for subsidized health insurance. By 2025, that figure had surged to approximately 477,838 individuals, demonstrating the positive impact of these programs. Consequently, the percentage of uninsured people in the state dropped from about 12% in 2020 to 8.8% in 2022, according to research from the Public Affairs Research Council of Alabama and the United States Census Bureau.
Smith emphasized the importance of the enhanced tax credits, stating, "These credits have been crucial in reducing the number of individuals trapped in the coverage gap. Our uninsured rate has improved significantly thanks to these enhanced premium tax credits."
